Dear Gunjan, Your question is not clear, if your ESIC company registration No. from delhi and your employee are working in Noida, Delhi, Gurgaon and Chandigarh then you need to take Sub Code for Noida, Chandigarh & Gurgaon then dispensary will be allotted region wise at the basis of your Company ESIC Sub Code. for some cases - if your employees is working in your delhi office and he is out of city, in case he goted any treatment there from medical Center then he need to fill form 105 to take his medical reimbursment from ESIC LO. form 105 is only for out side medical treatment.
