All the employees earning Rs.10000/- and less have to be covered under ESI.
Fillup the Employee registration form (available with the ESI office) and let the employee sign and then enclose 1 Post card size of each employee(if he/she wants her entire family to be covered, then the familily photograph of post card size) to be sumbitted. One more photograph should be pasted in note book and details mentioned and authorised by the HR and to be kept by the employee and he has to take this also to ESI hospital along with the ESI card.
